Program Hours: Monday - Friday, 8:00am - 5:30pm. Some Saturday service available. Call for details.
Intake/Application: Walk-in, phone for language and appointment availability.
Documents: Identification, insurance card, proof of income.
Fees: Low cost, sliding fee.
Method of Payment: Medi-Cal, cash, credit cards.
