Hours: Monday, Tuesday, Wednesday, Thursday 8 AM - 6 PM;
Friday 8 AM - 4:30 PM
Appointment preferred
Fees: Accepts Medicaid, Medicare, Insurance; sliding scale available
Must provide photo ID/driver's license, proof of income needed if applying for sliding fee scale
