Services Offered: Primary health care for adults and children, Dental Care, Well Women Exams and Pap Smears, Mental Health
Connectus offers Medical and Women's Health Care on a sliding fee scale to patients without health insurance who have income below 200% of the Federal Poverty Guideline. All patients are asked to complete a sliding fee application to determine the co-payment required.
New patients will need to show proof of income (e.g. pay stubs or letters of support from State agencies)
