This is a SLIDING SCALE clinic. The costs for clinic services are based on either your income or they offer type of financial assistance. Contact the clinic directly to discuss prices for individual services which vary. Sliding Scale does not necessarily mean free.
Foremost Family Health Centers fees are discounted to all qualified patients. Specific conditions must be met and all appropriate documents must be submitted. Sliding fee calculations are based on income and size of household. If you do not qualify for the sliding fee discount, or do not have all documentation, you will be responsible for paying full price. We accept cash, check and credit cards.

Increase percentage of patients who receive preventive dental services

Conscious sedation technique to relax children during minor dental procedures. (The child is awake and conscious – just more relaxed and calm)
